About The Position

Experience : Minimum 1.5 years general nursing required. Travelers must have more than 2 years of experience.

License : Registered Nurse (RN) - Board of Nursing

Certifications : BLS required.

Must-Have :

Previous discharge planning experience.

Experience in Acute Care Case Management.

Proficiency in patient progression of care.

Experience in meeting with families to discuss care plans.

Familiarity with the EPIC charting system is required.

Description :

Case Managers provide care for patients across all acute care departments.

The RN Case Manager will predominantly remain in the same area throughout their contract, subject to new hires.

Typically, travelers are not expected to float to multiple areas during their contract.

Patient ratios are 1 Case Manager to 16-20 patients.

Support on the unit includes Case Management Assistants, Lead Case Managers, Social Workers, Complex Psychosocial Consultants, and Utilization Review RNs.

There is no on-call requirement.

Holiday expectations include working 2 out of 4 major holidays.

Time-off requests for up to 7 days are approved, but requests for full weeks off must be extended.

There are no requests for time off during specific high-traffic periods in August each year.

Scrub color should be navy blue or professional attire.
