Description :

Manager interviews required.
Unit cares for a variety of patients awaiting placement in the community.
Patients may present with heavy behavioral or psychiatric needs, alongside long-term care and skilled nursing facility requirements.
Security is present on the unit due to aggressive patients.
Typically, the patient population is non-acute without the need for special monitoring or IV access.
Occasionally, patients may be admitted for IV antibiotics, but they do not require frequent doses.
Mobility of patients ranges from independent transfers to those requiring assistance.
Care is provided under a nursing home model with a focus on fall prevention, skin integrity, and activities.
The unit is supported by an activities coordinator, CNAs, and expert care management staff.
Nursing care is less than acute level; vital signs are taken once daily, and full assessments are conducted weekly.
Patient ratios follow a team nursing approach.
RN ratios can be up to 1 : 9, with four RNs and one charge nurse unassigned.
CNA ratios can be up to 1 : 8, with a goal of 1 : 6-7.
Daily responsibilities for RNs include medication administration, dressing changes, and necessary charting.
Charting includes daily nursing notes, behavioral notes, and activity participation.
CNAs are responsible for daily ADL charting, meal intakes, and assisting patients with activities.
LPNs will manage medication administration and support patient care, including dressing changes and vital signs.
Support on the unit includes a CNA, an unassigned charge nurse, two dedicated social workers, and a case manager.
Phlebotomy services are provided for peripheral blood draws only.
Charting system in use is Cerner.
Floating is unlikely for LPNs, but RNs may float to other areas within their scope of practice if needed.
Weekend rotation every other week is expected.
Holiday expectations will vary as needed.
Time off approval will go through the manager.
Scrub color must be professional, and acrylic nails and Crocs shoes are not permitted.
