Position Summary : The Ohio Healthy Transitions Program (OHTP) coordinator is expected to support the activities directly related to improving access to treatment and support services for youth and young adults, ages 16-25, who have a serious emotional disturbance or serious mental illness .

This position will be involved with increasing awareness, screening and detection, outreach and engagement, referrals to treatment, coordination of care, and evidence-informed treatment.

Responsibilities Include : Participate and lead outreach to Transitional Aged Youth and their families; serve as an ambassador of the youth and their families to navigators and programs Work with individuals and groups in the community to determine the needs of youth and young adults.

Identify the needs and interests of the youth and young adults and find resources and organizations to help them Establish an advisory council across youth and adult serving systems that is inclusive of key decision-makers.

Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
