Job Summary :
The Chronic Registered Nurse is responsible for providing and coordinating clinically competent care for a designated group of patients with End Stage Kidney Disease (ESKD). This includes assessment, planning, implementation, and evaluation, along with appropriate delegation to licensed and unlicensed direct patient care staff. The RN plays an integral role in patient education, care quality, and facility operations.
Key Responsibilities :
Deliver all essential nursing functions under the direction of the Supervisor and Clinical EducatorConduct ongoing assessment and documentation of patient status throughout hemodialysis treatmentsAdjust care plans and notify supervisory or medical staff as neededEducate patients and families regarding care, treatment options, and transplant opportunitiesAdminister medications per prescription and document justification and effectivenessLead or assist in emergency response situationsSupport staff training and orientation of new team membersOversee proper lab collection, processing, and shippingUpdate the nursing assessment and plan of care through collaboration with the interdisciplinary teamPerform additional duties as assigned by the SupervisorRequired Experience :
1 year of Nephrology Nursing experience within the past 2 yearsChronic hemodialysis experience requiredNice-to-Have Experience :
Experience as Charge Nurse or Team LeaderFamiliarity with transplant education for dialysis patientsRequired Skills :
