Breakdown of case mix? Ortho / urology / eyes / endo

Patient demographic : Adults / peds : dental eyes only

Facility will reimburse for the following costs : Flight, Hotel, Rental Car

• Spinal Anesthesia
• Epidural Anesthesia
• Regional Anesthesia
