TravelNurseSource is working with Patterns LLC to find a qualified Home Health RN in Broken Bow, Nebraska, 68822!\n\nPay Information\n$30 to $45 per hour\n\nAbout The Position\n\nSIGNIFICANT DUTIES\nInitiates assessment / reassessment based on the patient's needs per nursing standards and organization policy and procedures.\nSynthesizes pertinent patient data to develop nursing care plans and identify nursing priorities for care and desired outcomes.\nCollaborates with the interdisciplinary team to coordinate and provide patient care.\nDemonstrates knowledge and skill in nursing theory and ability to learn and develop competence in patient care across the lifespan.\nDemonstrates the ability to learn new practices and has the ability to practice in multiple settings within the rural hospital - medical / surgical, pediatric, neonatal, emergency and skill care.\nIntegrates appropriate standards of care into the patient's plan of care based on the diagnosis and age-specific needs.\nExecutes, directs, supervises and / or assigns other staff members to perform therapeutic and or supportive interventions.\nEvaluate the patient's response to and effectiveness of interventions.\nEvaluates the patient's progress toward established outcome.\nIncorporates cultural considerations into the plan / delivery of care.\nIncludes family and / or significant others in the care of the patient as appropriate.\nDocuments patient care and related information according to Melham procedures for documentation\nFacilitates a patient's transition to the next level of care including admission, discharge, and transfers.\nReceives calls for patient placement, screens for appropriate admission criteria and notifies the department of patient placement.\nUtilizes knowledge of UR criteria to assist with the appropriate admission status and placement of a patient.\nDemonstrates knowledge of EMTALA regulation and paperwork.\nAssumes responsibility in a shared or delegated assignment and supports team member's practice through timely communication.\nServices as a role model to colleagues, students, and other care providers\nActively participates in continued learning opportunities and continued self-development pertinent to all areas of Acute / ED care nursing.\nProficient in the management of provider orders in CPOE and in the order management system\nMaintains effective communication channels with the entire healthcare team.\nAssist administration with personnel management and other administrative duties as assigned.\nActs as a resource to the healthcare team\nPerforms as senior leader in event CNO / nursing management are absent.\nPerforms other related duties as assigned.\nOversees and / or assigns additional work duties during low volume periods when core staffing is in place.\n\n27188024EXPPLAT\n\nJob Requirements\n\nQUALIFICATIONS\nGraduate of School of Nursing; BSN or BA preferred.\nMinimum of 6 months of clinical experience, work related experience and / or previous House / Shift Supervisor / management role in a hospital setting. and ability to perform duties in acute, skilled, and emergency areas of facility.\nCurrent NE State License or Compact license as a Registered Nurse required.\nProfessional knowledge of nursing practice, and regulation to support patient care delivery.\nCurrent BLS, ACLS, PALS or ability to obtain within 2 months of hire.\nCurrent TNCC or ability to obtain within 1 year of hire.\nIS 100, 200, 700 and 800 or ability to obtain within 12 months of hire.\n\nGENERAL REQUIREMNENTS\nDemonstrate commitment to the organizational mission, vision, and core values.\nMaintain clinical competency as required for Acute and Emergency Room Care including but not limited to age-specific competencies relative growth and developmental needs of all ages of patients.\nKnowledge of federal and state regulations related to nursing applications and relative practice area.\nCommunicate effectively both verbally and in writing.\nSupervise, instruct, and evaluate the work of all acute and ED patient care staff members.\nMaintain confidentiality of patient information\nExercise professional judgment, courtesy, and professionalism in daily interactions with others\nDemonstrates relationships management and influencing behaviors.\no Conflict resolution skills\no Critical conversation skills\no Mediation skills\nDemonstrates knowledge of Melham Medical Center Compliance Program, Code of Conduct, and Confidentiality standards. Monitors staff adherence and knowledge of standards.\nServes as the lead RN for emergency management plan response. Opens incident Command as appropriate. Monitors staff knowledge and response to plan activations.\nDemonstrates knowledge of risk management, quality review reports, Sentinel Events, near misses and non-punitive work environment concepts.\no Incident reporting SQSS\no Compliance and safety reporting\no Abuse / neglect reporting\no Patient rights\no Staff rights\no Appropriately completes and / or supports reporting of events and forms as needed.\nCompletes yearly self and staff evaluations / reviews and development plan.\nAttends meetings as assigned.\nAttends 75% of monthly staff meetings and keeps updated on information.\nCompletes annual on-going competency plan.\nAttends mandatory in-services and / or programs.\nMaintains certifications.\n\nAbout Patterns LLC\n\nPatterns is one of the leading service providers in the business process outsourcing industry.\n\nPatterns was incepted with the pursuit of being the one-stop solutions provider for all the business process outsourcing needs.
