JOB SUMMARY

Telephone triage is the process of collecting information over the telephone to determine the urgency of a health problem, the type of medical intervention necessary and how soon treatment should begin.

This same assessment may be necessary for "walk in" patients who present to the Health Center.

ESSENTIAL JOB DUTIES

Uses triage protocol effectively by determining urgency category :

a. Emergent patient is seen immediately or instructed to call 911 or go to the Emergency Room at local hospital.

b. Urgent patient is seen on an urgent basis the same day or the earliest next day appointment.

c. Non-urgent patient is seen at the next available appointment or within 2-3 working days.

Completes Electronic Health Record to document patient complaint, symptoms, and / or other information received from the patient or caller.

Documents protocol followed, advice given and / or resolution to the problem / complaint.

• Refers calls or consults with providers and nursing staff as appropriate.
• Uses effective public relations skills when interacting with patients and callers. Maintains professionalism and supports the mission of GLBHC at all times.
• Provides education and resources to support the patient. Assures appropriate needs are met through assessment of patient's level of understanding and ability to implement any follow up plans.

MARGINAL JOB DUTIES

• Gathers or maintains statistical data regarding triage calls (information regarding number of calls, peak call times).
• Provides input to provider schedule changes.
• Assists with patient processing / care delivery as able.
• Other duties as assigned.

JOB SPECIFICATIONS

• Education : Graduate of an accredited school of nursing.
• Licensure : N.
• Experience : Two years minimum experience working in a family practice setting. Triage experience preferred.
• Skills : Excellent assessment skills. Able to gather data that is accurate and precise. Records and documents data clearly and objectively.

Positive public relations skills. Use of good judgment and ability to effectively communicate plan of care.

• Interpersonal skills : Able to communicate effectively with a diverse population. Bilingual preferred.
• Physical Effort : Low
• Hours of Work : Full time, flexible, and varied.
• Travel : Minimal travel required. Mileage reimbursement in accordance with existing rate of the corporation.
