Hospice Medical Director with Sentara in Virginia

Sentara Medical Group is searching for a Hospice Medical Director. This position will provide leadership, oversight and guidance for multiple hospice programs (only for Sentara Hospice) throughout Virginia.

Position Highlights :

• Collaborates with the hospitals, medical group, palliative, and hospital medicine to ensure a smooth transition of care for patients.
• Participates in weekly Hospice Interdisciplinary Team (IDT) meetings
• Develops and oversees end-of-life care of patients and their families through clinical programs and pathways
• Participates in quality improvement initiatives and program development
• Assists with development and implementation of education for members of the Hospice Interdisciplinary team and community
• Serve s as a hospice champion, promoting and representing programs to physicians, discharge planners, other referral sources and community health organizations as appropriate.
• Teaching opportunities with local medical students, Residents and Fellows
• Continuing medical education - need to verify with Kim Bradley / Keren Ellis

Duties include :

Administrative Medical Oversight of Hospice team

Determining Hospice eligibility

Certification of terminal illness

Interdisciplin ary meetings

Medication utilization oversight

Completing required documentation to support end-of-life care

Mon - Fri 8am - 5pm

Home call : 1 week per month, including 1 weekend as back up to NP

Minimum Qualifications for this opportunity :

• Licensed to practice in the Commonwealth of Virginia.
• Prior Hospice experience preferred.
• Prior leadership experience preferred.
• Ability to attend 50% of IDT meetings
