GENERAL DESCRIPTION OF THE POSITION :
The Behavioral Health Care Coordinator serves as the first point of contact for new patients and their families. This role is responsible for creating a welcoming, supportive environment, guiding patients through the admissions process, and ensuring all necessary pre-screening and intake steps are completed accurately and efficiently. The Care Coordinator helps bridge the gap between patients, providers, and the treatment team by facilitating communication, documentation, and coordination of services. ESSENTIAL FUNCTIONS :
1. Patient Welcoming & Onboarding
- Greet and welcome new patients.
- Conduct tours of the facility to familiarize patients and families with available services, staff, and treatment environment.
- Serve as a supportive point of contact for patient questions during the admissions process.
2. Pre-Screening & Admissions
Complete pre-admission screenings and gather necessary documentation.Review intake forms, verify insurance information, and ensure all admission requirements are met.Assist patients and families in understanding program expectations, policies, and treatment options.Coordinate with clinical staff to prepare for new patient admissions.3. Care Coordination & Administrative Support
Maintain accurate and timely documentation in the electronic medical record (EMR).Support smooth scheduling and follow-up for assessments, evaluations, and first appointments.Communicate effectively with clinical and administrative teams to ensure seamless patient handoff into treatment.Provide assistance with referrals and linkages to additional community or internal resources as needed.ADDITIONAL FUNCTIONS :
Performs other duties as may be assigned.Follows and supports TPEG policies and procedures.Works collaboratively and cooperatively with internal and external partners.Represents TPEG, Inc., by embodying the mission, vision and values of the organization.Travels occasionally during the workday and on occasional overnight stay. Requirements :
MINIMUM QUALIFICATIONS :
EDUCATION : Bachelor’s Degree in Psychology, Social Work, Behavioral Health, or a related field required.RELATED EXPERIENCE : One (1) year of experience in healthcare, or mental health.OTHER EXPERIENCE / SKILLS REQUIRED : Strong interpersonal and communication skills, with the ability to engage patients and families in a supportive manner.Detail-oriented with strong organizational and time management skills.Experience working with culturally diverse individuals and communities or have otherwise demonstrated a commitment to strengthening engagement of a diverse community and skill in communicating with diverse populations.Familiarity with equity and / or diversity initiatives within an organization.Written and oral communication skills sufficient to perform essential functions.Demonstrated ability to make decisions independently and use initiative to accomplish complex assignments with general instruction and guidance.Demonstrated ability to exercise judgment and integrity on the job with confidentiality, tact, and diplomacy.Demonstrated ability to organize time and other resources to perform multiple tasks.Demonstrated ability to complete work accurately and in a timely manner with attention to detail.Demonstrated ability to work well with others and to provide effective team leadership.Proficiency in word processing and database and / or spreadsheet applications.Physical and mental attributes sufficient to perform essential functions.Demonstrated ability to recognize merit, excellence and intelligence in staff and potential employees.Valid Driver's License / Clean Driving RecordPREFERRED QUALIFICATIONS :
