Job descriptionEmployment Type : Full time Shift : Day Shift Description : Shift : M-F 8a-4p St. Peter's Hospital in Albany, NY is looking for an experienced RN to work as a Registered Professional Nurse as part of the C3 Patient Logistics team full time days. The team is willing to help train the right candidate as long as they meet the minimum requirements and have the desire and ability to learn and grow into this wonderful opportunity to progress in their nursing career. As a member of our caring, compassionate health care professionals the selected RN will enjoy : Great shift differentials Be part of a grow team spanning the Capital Region Enjoy the benefits of the national Trinity Health Care organization while maintain the personal relationships of those in the community and the standards of St. Peter’s Health Partners Leadership team that takes input from the staff to help with process improvement Easy commute from Troy, Guilderland, Latham and Colonie Thorough orientation to ensure success The selected RN will : Receive calls throughout our organization and externally to get patient into the facility Triage calls Problem solve and utilize critical thinking skills to deescalate and maintain customer service and patient satisfaction Work closely with the Case Management team to insure the patients are processes timey while maintaining accuracy and patient satisfaction Travel to Samaritan and Albany Memorial Hospitals as needed primarily during orientation Minimum requirements : Current and valid NYS RN license Bachelor of Science in nursing, or willingness to purse defined plan for matriculation in program demonstrated by ongoing course work each semester 3 years of experience in acute setting (critical care preferred) Can orient during the week Monday – Friday on a flexible day schedule Preferred requirements : 1 year of experience in case management or utilization management Case Management certification (CCM) or Certified Professional in Healthcare Management (CPHM) Discharge planning and community resource knowledge Maintain the highest level of customer service while completing day to day duties and responsibilities Critical thinking skills to problem solve Excellent time management and organizational skills High level of efficiency to communicate in a fast pace work environment across people and teams of different backgrounds and demographics Engage within a team setting to help promote a strong culture Passion to learn, grow and develop in the nursing field Pay Range : $33.10 - $45.75 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
