Community Health Worker - Supportive Housing Services Adult System

Job Summary : The Community Health Worker-Supportive Housing Services Family System will be responsible for community outreach, navigation, and supportive housing services to the clients of East County Housing, this includes families with at least one minor.

The CHW builds relationships with homeless neighbors, provides advocacy, and assist with connecting individuals experiencing homelessness with services and housing.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Outreach tasks include but are not limited to :

  • Requires willingness to travel and engage with community members, occasionally including some weekends.
  • Participate in outreach efforts in a variety of settings (e.g. street outreach, encampments, through partner organizations and agency referrals, and transitional and emergency shelters) that takes this position outside of the controlled work environment, including unpaved paths.
  • Initiate contact with unsheltered community members to develop relationships through frequent outreach visits to foster trust building.
  • Identify and assist eligible community members with navigation of appointments and follow-ups to ensure successful utilization of program resources.

Navigation tasks include but are not limited to :

o Assist clients with accessing resources and referring to appropriate resource

o Identify appropriate permanent housing options for clients

o Assist clients with housing applications, completing supportive and subsidized housing paperwork, surveying the rental market for affordable housing, and advocating for clients with prospective landlords

o Identify employment opportunities for clients

o Cultural mediation among communities and the Social Service Systems

Supportive Housing Services tasks include but are not limited to :

o Coaching and social support :

  • Confer about obstacles preventing participation in programs and identifying means of overcoming these obstacles.
  • Advocate for clients as they navigate systems.

o Care coordination

Encourage and support client participation in activities and events that increase ability to remain linked to care and / or behavioral health care.

Case management

  • Develop a case plan with short and long-term goals that address barriers to obtaining services and / or housing.
  • Monitor and evaluate each client's progression toward their goals and modify the plan if necessary.
  • Maintain computerized communication, client notes, and other reports Store, maintain, and protect the integrity and security of community members’ files
  • Process paperwork according to HIPPA guidelines, and maintain all information securely and confidentially

Provide consistent record keeping, documentation, and communication to all clients assisted.

Qualification Requirements

To perform this job successfully, an individual must be able to perform each essential duty

Drive to appointments to assist clients; a valid driver’s license, reliable vehicle, and

insurance is required. Mileage reimbursed by Rockwood CDC.

Provide supportive case management services to shelter families or houseless

individuals to obtain permanent affordable housing, secure financial stability, and

achieve self-sufficiency.

  • Excellent organization and prioritization, attention to detail, and multi-tasking abilities.
  • Excellent communication and active listening skills that include cultural responsiveness.
  • Ability to work well independently and with others in the office and in the community.
  • Knowledge of computers and applicable computer programs such as Microsoft 365;

must be able to pass basic computer proficiency test.

  • Understanding of the community development process.
  • Ability to find and leverage community resources.
  • Ability to maintain confidentiality when working with sensitive information.

Education and / or Experience

  • Minimum of High School Diploma or equivalent.
  • Minimum of 2 years of case management experience.
  • Experience working in a community-based organization.
  • Must attain and maintain an OHP Assister certification.
  • Must maintain and / or obtain Community Health Worker certification within six months

of hire.
