Position Summary
Under the sponsorship of Hamilton-Madison House, the Social Adult Day Services Program provides services with the goal of improving the quality of life for older adult aged 60 and over with dementia or related cognitive impairments by providing meaningful social activities, based on the assessment of their strengths and interests, which will engage them, and promote self-esteem and a sense of achievement.
The Program Coordinator provides supportive services to participants and takes care of the daily program and administrative operation of the Adult Day Services Program. The coordinator also works cooperatively with other staff, caregivers, and outside agencies to assure maximum continuity and quality of the program—reports to the Director of Two Bridges Housing.
Minimum Qualifications
Bachelor’s degree / associate degree with minimum 1 year experience in the human services field
Strong commitment to the mission of the Social Adult Day Services Program
Nonjudgmental, socially perceptive, empathic, and a good listener
Sensitivity to and understanding of the needs of the elderly. Prior experience working with older adults is desirable
Highly organized, detail-oriented; able to plan, manage, and complete multiple concurrent projects, frequently with short deadlines
Ability to handle complex and difficult situations in a professional manner.
Demonstrated ability to conduct oneself in a calm and professional demeanor when dealing with the public and / or with difficult situations
Ability to develop collaborative relationships with professional staff and community groups
Excellent verbal and written communication skills
Understanding and appreciation of the role of race, religion, ethnicity, sexual preference, and individual values as it relates to serving clients and their families.
Commitment to diversity and ability to work with a diverse population of seniors, volunteers, and staff
Experience in handling health and safety situations related to older adults.
Computer skills including Word, Excel, and PowerPoint
Completion of Food safety certificate required
Update CPR and First Aid training
Bilingual required : English and Chinese (Cantonese Preferred)
Responsibilities
Creates and contributes to the friendly, warm, and safe environment of the program
Treats other staff, clients, and caregivers with dignity and respect
Adheres to the Client Bill of Rights and Hamilton-Madison House’s Code of Conduct
Participates in outreach events in the community
Plans and develops social, educational, recreational, and health promotion activities
Assists participants with daily living skills, such as feeding, toileting, grooming, and transferring. Provides individualized support for these tasks based upon the needs of each client and in keeping with each client’s plan of care
Conducts in the new intakes and assessments required of participants to attend the program
Accesses and inputs into the PeerPlace system for the Department for Aging
Enters data and keeps accurate statistics / records as required including observing and documenting participants' daily activities
Generates monthly invoices and learns the Daycare software
Oversees and helps maintain a tidy and safe program area, ready for use by program participants
Notifies the program supervisor of any changes, issues, or observations concerning clients and caregivers
Ensures program’s compliance with government regulations
Attends daily and monthly meetings as scheduled
Attends and participates in orientation, staff education, and ongoing training
Handles Para-transit services and needs by coordinating schedules and transport services for all Senior Services programs including interface with the Para-transit Driver
Oversee 2 part-time Program Assistants, Interns and Volunteers
Compensation : $33 per hour
Hours : Monday to Thursday, 9 : 00AM to 4 : 00PM, 30 hours per week
