Position Summary :
We are seeking a skilled and motivated Speech-Language Pathologist to join our home health team on a PRN basis. The ideal candidate will provide assessment, therapy, and education to patients experiencing speech, language, cognitive, or swallowing difficulties due to injury, illness, or disability.
Responsibilities :
- Review request for speech therapy and physicians' orders.
- Evaluate type of therapy most desirable after consulting with physician.
- Interview the patient to determine type of treatment and therapy schedule.
- Explain procedures involved in speech therapy to the patient and prepare necessary equipment.
- Demonstrate to patients, family and staff personnel, as necessary, the use of equipment involved in therapy.
- Prepare patient for treatment by dress / position and administer speech therapy in accordance with established policies and procedures.
- Recommend modifications or changes in the patient's therapy program based on own evaluation of progress.
- Reassure patient before and during therapy treatment.
- Assure that all clinical progress notes are informative and descriptive of the care provided and of the patient's response to the care.
- Participate in the development and implementation of patient care plan, including regular case conferences.
- Performs other duties as assigned.

Requirements :
Possesses a degree that includes an internship from an approved masters program in Speech–Language Pathology, or as required by applicable law and / or regulation.Meets the education and experience requirements for a Certificate of Clinical Competence in Speech–Language Pathology, granted by the American Speech–Language–Hearing Association (ASHA).Is certified to practice Speech–Language Pathology.A minimum of one (1) year's clinical experience in Speech–Language Pathology.Demonstrates good verbal and written communication, and organization skills.Possesses and maintains current CPR certification.Must be a licensed driver with an automobile that is insured in accordance with state / or organization requirements and is in good working order.Understands home health philosophy and focus of patient care with attention to attainment of patient centered goals.Must Show Proof of a Tuberculin Skin Test (PPD), TB blood test, and / or CXR, or get one upon employment (An annual assessment for symptoms must be completed thereafter). Proof of immunizations for MMR (X 2 if born after 1957), D / T and Varicella is recommended, but not required.Annual immunization against influenza required.
