Overview

The Senior E-Referral Specialist will serve as team lead responsible for the day-to-day performance of an area, managing referral receipt, workload, and timeliness as well as participating in referral processing. The e-referral team is responsible for receiving and responding to community, physician, patient and family request for home health. This position will record and process referral information as well as document in patient information databases and act as the critical liaison between referral sources and our sales and operations teams. The Senior E-Referral Specialist will train, schedule, and mentor junior teammates and assist with management projects that improve the effectiveness and efficiency of the portal team. Manage incoming referrals.

Responsibilities

• Perform required analysis, decision-making, and data entry accurately and timely.
• Assist in team lead and management duties.
• Patient information and forms are managed and forwarded to the appropriate department per procedure.
• Monitor all assigned portals for new referrals, messages, bookings, and updates.
• Work with the intake department and local branches to ensure communication of all booked referrals, referral sources messages and updated documents coming through the portal and to

minimize any delays in care of our patients.

Qualifications

Associate degree preferred; High School Diploma required.
