About The Position

Must-Have : Proficiency in OASIS and HomeCare HomeBase within the last 6 months

Description :

Provides coordination and direction for the provision of home nursing care according to physician's orders.

Utilizes agency policies and procedures through competent application of the nursing process.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.

Conducts home visits and attends periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average patient load is 5-10 patients per day.

Each type of visit and activity holds a different point value, totaling approximately 30 points per week.

The RN will manage various types of cases, including Start of Care, Wound Care, and Follow-up Visits.

On-call rotation is required every three weeks based on staffing needs.

Willingness to travel up to 75 miles from the designated service area is necessary.

Mileage is reimbursed at the IRS standard rate.

Documentation must be completed within 24 hours of patient appointments.

Failure to accept assigned patients may result in cancellation.

No guaranteed hours are provided; meeting weekly productivity points of 30 is expected.
