Physical Therapist

Job Description

This position is located at our Woodland, Gibson Road location.

POSITION SUMMARY : Under general supervision from the Lead Medical Provider, the Physical Therapist is responsible for managing referrals from both internally generated patient care as well as external patient care providers. A successful incumbent will work as part of a care team for NVIH patients by collaborating with medical providers and support staff to create an environment to help patients improve through physical therapy techniques.

ESSENTIAL JOB FUNCTIONS : The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) will be required to perform a variety of duties listed, including but not limited to :

• Provide direct physical therapy services to program patients, including, but not limited to : obtaining / reviewing and documenting appropriate patient history, making and documenting diagnoses / assessment, develop and document an individualized treatment plan, assess and document progress, and communicate appropriately with referring providers.
• Maintain equipment and work area in clean and safe condition.
• Initiates physical therapy treatment plans and instructs other personnel and caregivers, as appropriate
• Treat patients in a Respectful and Compassionate manner to motivate and inspire them to continue with treatment plans for improved health outcomes.
• Assisting with development of programs and policies relating to physical therapy.
• Participate and assist in quality assurance and peer review processes as directed.
• Other duties as assigned.
