Job Description

As a Palliative Care Nurse Practitioner, you will provide compassionate care to patients facing chronic, complex, and life-threatening conditions. Your role involves collaborating with a multi-disciplinary team to enhance the quality of life for patients and their families. Here are the key responsibilities :

• Patient Assessment and Management :

Conduct thorough assessments of patients physical, emotional, and psychosocial needs.

Work closely with a diverse team, including case workers, chaplains, and hospitalists.

Provide disease and treatment education to patients and their families.

This position operates during regular business hours, Monday to Friday, from 8 : 00 am to 5 : 00 pm.
