Job description
A company is looking for a Bilingual Care Coordinator to support care coordination functions and advocate for members' health needs.
Key Responsibilities
Conduct health risk assessments through member outreach to understand their health and needs
Coordinate with provider offices and home health agencies to ensure members receive necessary support
Participate in interdisciplinary rounds and review care plans to provide additional support to members
Required Qualifications
Minimum education : High school diploma or GED equivalent
2+ years of experience in healthcare administration or home care
Proficient in technology (experience with Gmail / G Suite)
Previous experience in social services
Fully bilingual (English / Spanish)
