A leading healthcare provider in the Central New York area is currently seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join their team. The successful candidate will support the Independent Practitioner Panel (IPP) in issuing physician orders for individuals eligible for community-based long-term care services.

Responsibilities :
Reviewing and recommending consumer assessmentsMaintaining knowledge of regulatory changes and clinical qualityValidating assessment outcomes using clinical skillsCollaborating on quality assurance and improvement initiativesSupporting program goals through education and communicationParticipating in internal meetings and performance reviewsPerforming other duties as assigned by managementConducting interview-style assessments (no physical / vitals required)Qualifications :
2+ years as an Advanced Practice Provider (NP or PA)Master's Degree from an NP or PA ProgramActive New York State LicenseActive Medicaid NumberBLS CertificationExperience with adult / geriatric populationsComputer literacy and tech navigationStrong written / oral communication skillsInterpersonal skills and adaptabilityDesired Skills :
Internal Medicine, Adult Health / Geriatrics, Family Practice, Community Health specialtiesKnowledge of long-term care programsProficiency in languages such as Spanish, Russian, Haitian-Creole, Chinese, Korean, and / or Bengali
