Job Description
The Medication History Specialist supports accurate medication reconciliation for new patients admitted at their hospital site.
Assists the entire patient care team (pharmacist, nurse, provider, etc) in ensuring the completion and accuracy of medication histories.
Medication history documentation responsibilities will be conducted in various patient care settings including but not limited to : emergency department, inpatient care nursing units, surgical pre-admission testing office, or any other place at the respective site assigned by the pharmacist, provider, or supervisor.
To ensure the most accurate information, medication documentation involves collecting information from various sources including : the patient, patient caregiver, any available health information exchange(s), retail pharmacies, physician offices, or any other source as directed by the pharmacist, nurse, provider, or supervisor.

Minimum or Preferred Qualifications
HS Diploma or GED required. AAS in Business / Healthcare-related degree preferred. BA / BS in Business / Healthcare-related degree preferred.
3 years of experience in a healthcare setting (within the past 5 years) preferred. 1 year of direct patient care in a healthcare setting preferred.
Typing skills assessment required.
You must meet one of the below requirements :
- Certified Pharmacy Technician (PTCB certification)
- Certified Medical Assistant
- Relevant NYS Healthcare License (such as but not limited to LPN)
- Actively enrolled in one of the following healthcare degree programs at an accredited institution : Nursing (Nurse / Nurse Practitioner), Pharmacy (Pharmacist), Medicine (Physician), Physician Assistant, Dentistry (Dentist), Dietitian / Nutritionist, Physical Therapist
Salary Range
$22.84-$28.92 per hour *Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.*
