Job Summary : In collaboration with the cardiologists, provides coordination of disease management activities in the outpatient ambulatory settings for a cardiac patients for the purpose of primary and secondary prevention.

Triages patient and provider calls. Is a resource for staff in the management of this patient population. Coordinates care to ensure safe, effective, efficient patient centered care.

Essential Responsibilities :

Provides front line support to patients and staff in Outpatient Cardiology, including coordination of disease management activities in for primary and secondary prevention cardiology patients.

Collaborates with physicians, nurse practitioners, hospital clinical staff, and the patient to coordinate necessary aspects of post-hospitalization care including : medication reconciliation and scheduling of follow up cardiology appointment.

Triages patient and provider calls including symptom targeted phone assessments, coordination and follow up of laboratory tests, refilling cardiac medications, and consultation with clinical staff when needed.

Acts as resource to the administrative and clinical staff to assist in the management of patient care and scheduling issues both in the ambulatory setting.

Provides education to both the cardiology nurses as well as patients on cardiac disease management, pharmacology, and common patient issues.

Uses good clinical judgement and decision making skills.

Updates clinical knowledge and skills through consultation, formal and informal, education and review of literature.

Participates in the collection of statistics related to heart failure patients) . readmission rates,

Identifies process improvement opportunities related to the general cardiology and heart failure patient population and participates in the improvement initiative.

Works effectively with all disciplines to ensure the best outcomes for patients.

Required Qualifications :

Associate's degree required. Bachelor's degree in Nursing preferred.

License Registered Nurse required., and Certificate 1 Basic Life Support required.

5-8 years related work experience required.

Must have a sound understanding of biological, social, and behavioral theory. Sound base of scientific knowledge and physical assessment skills required for protocol implementation.

Excellent communication and organizational skills are required along with being detail oriented.

Proactive individual capable of independent decision making and the ability to work collaboratively with others in a variety of settings and within an extended regional area.

Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

Preferred Qualifications :

Cardiology experience.

Knowledge : Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.

Team Work : Ability to interact respectfully with other employees, professional staff and / or external contacts to offer ideas, identify issues, obtain information or deliver services.

Customer Service : Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving.

Ability to remain calm in stressful situations.

Physical Nature of the Job :
