Position Overview

We are seeking a compassionate, experienced, and self-motivated Physical Therapist to join our team in a part-time, school-based role supporting children with special healthcare needs. This role is critical in addressing a care gap for pediatric clients with chronic musculoskeletal and neurological conditions. You will work closely with a supportive clinical team in a setting focused on long-term, ongoing therapy rather than curative or acute rehabilitation.

Location : Hanford, CA

Schedule : Part-time | 20 hours per week, no weekends, no holidays, prefer one person who can cover for continuity but will consider 2 people who can rotate with each other.

Key Responsibilities

• Deliver physical therapy services to children and young adults aged 3 to 21 with chronic care conditions.
• Develop and implement individualized treatment plans focused on ongoing therapy needs .
• Collaborate with interdisciplinary team members including nurses, office staff, and school personnel.
• Maintain accurate documentation using e-Clinical , the organizations electronic record system.
• Participate in onboarding and unit orientation; complete self-guided study of DHCS policies and documentation standards.
• Manage caseloads in accordance with Title 22, Section 41517.5 of the California Code of Regulations.
