Provide community-based Blended Case Management services to children and adults in Central Bucks CountyMeet individuals and family members in their homes and communities to assess needs and connect them to vital services, resources and programs to positively impact an individual's overall well-beingCollaborate with LVF's multidisciplinary team and external providersMaintain accurate, recovery-oriented documentation in our Electronic Health Record (EHR)About Lenape Valley Foundation
