Registered Nurse (RN) - Full-Time PM Shift

Sierra Valley Rehab Center is seeking a dedicated and compassionate Registered Nurse (RN) to join our team on the Full-Time PM Shift . The RN plays a key role in supervising day-to-day nursing activities, ensuring regulatory compliance, and maintaining the highest standard of resident care.
General Purpose
The primary responsibility of the Registered Nurse is to supervise and support the nursing activities of the facility during the assigned shift. This supervision must comply with all federal, state, and local regulations governing long-term care facilities. The RN may also perform duties as directed by the Director of Nursing (DON) and / or Assistant Director of Nursing (ADON) to ensure quality care and safety for all residents.
Essential Duties

Administrative & Supervisory Responsibilities
- Assist the Director of Nursing Services in directing and overseeing day-to-day nursing functions.
- Help develop, update, and implement nursing policies, procedures, and manuals.
- Ensure nursing staff follow all procedures, standards, and documentation guidelines.
- Provide timely written and verbal reports to the Director of Nursing.
- Participate in quality assurance, inspections, and resident care planning.
- Conduct shift rounds to ensure all nursing staff perform according to acceptable nursing standards.
- Ensure adequate staffing of licensed nurses and CNAs for the PM shift.
- Assign, delegate, supervise, evaluate, and support nursing staff.
Nursing Care Responsibilities
Admit, transfer, and discharge residents in accordance with facility procedures.Complete daily rounds to assess and document resident conditions.Provide direct nursing care as needed, including IV starts, treatments, and specimen collection.Ensure physician orders are followed, documented, and communicated.Monitor medication administration and treatment schedules for accuracy.Communicate with physicians, families, and other stakeholders regarding resident status.Respond promptly to emergencies and changes in resident condition.Care Plan & Documentation
Participate in developing and updating individualized care plans.Review and ensure complete, accurate, and compliant nursing documentation.Ensure nursing personnel follow the resident's care plan at all times.Safety & Infection Control
Ensure compliance with infection control, sanitation, and universal precaution protocols.Ensure proper use and availability of PPE.Participate in fire safety, disaster preparedness, and OSHA-required training.Report hazardous conditions or exposures promptly.Qualifications
Current, unencumbered California RN license .Minimum 2 years of experience preferred; LTC experience highly valued.Strong leadership, communication, and decision-making skills.CPR certification required.PCC experience preferred.Physical Requirements
Ability to move intermittently throughout the workday.Ability to lift / push / pull at least 25 lbs and assist in resident lifts / transfers.Ability to work with elderly, ill, or emotionally upset residents.Work Environment
Frequent interaction with residents, families, medical personnel, and staff.Exposure to infectious waste, diseases, odors, and hazardous chemicals may occur.May require occasional weekends, holidays, or emergency call-ins.Equal Employment Opportunity (EEO)
