Position Description :
A Wound Care Nurse plays a crucial role in a skilled rehab nursing home by overseeing and coordinating all aspects of wound care for residents. This position requires a highly skilled and knowledgeable individual who can provide comprehensive wound assessment, treatment, and education to promote optimal wound healing and resident well-being. A Wound Care Nurse collaborates with the interdisciplinary team, including physicians, nurses, therapists, and other healthcare professionals, to develop and implement effective wound care plans. Additionally, a Wound Care nurse stays up-to-date with the latest advancements in wound care management and ensures compliance with regulatory standards.
Responsibilities of a Wound Care Nurse (RN) :
Responsible for overseeing the Wound Care MeasuresMust have up-to-date MMR vaccination & physicalResponsible for overseeing the infection control programCommunicate changes in residents' conditions to IDT and other partners as appropriateParticipate in the on-call rotation with other nursing supervisorsFlexible working hoursAttend Quality Measures meeting and report on applicable informationAttend and prepare for weekly Case Management meetingsIncludes but not limited to items listed on the Registered Nurse Job DescriptionAbility to educate partners by conducting in-services and one on one learningAbility to investigate, track and trend identified areas of opportunitiesDevelop action plans for areas of needed improvementProvide partner support and coachingPassion for wound care and healing residentsPassion for infection control measuresRequirements of a Wound Care Nurse (RN) :
