Project Coordinator
Under general supervision, the class coordinates the administration, development and implementation of a specific project or program moderate in size and scope for a City department; and performs related duties as required.
Essential duties include :
- Supporting Senior Services including, but not limited to the Caregiver Portfolio, Dementia Initiatives, and other programs.
- Conducting and convening community outreach events with DFSS staff, DFSS delegate agencies, and other community partners.
- Establishing awareness of Senior Services and maintaining rapport between community institutions such as Regional and Satellite Senior Centers, libraries, aldermanic offices, hospitals and DFSS.
- Attending community meetings and serving as a liaison with City departments, public and private agencies and business organizations.
- Supporting Delegate Agencies with caregiver program implementation.
- Serving in special committees related to areas of concern of and protection for caregivers, older adults and individuals with disabilities.
- Developing, overseeing marketing strategies to encourage participation in related projects and programs.
- Communicating with caregivers or older adults and / or professionals who may be seeking resources.
- Contacting existing and potential participants for recruitment, retention, and follow up.
- Referring clients to information and assistance team to link to other services.
- Completing data collection and entry on Salesforce / Chi311 App, AgingIS, and TCARE and preparing periodic reports on activities, status, and progress of funder metrics and outputs.
- Administering screeners, assessments and monitoring data.
- Inputting, approving Delegate Agency vouchers for payment and entering APS and EA entry and tracking.
- Providing SHIP information and Medicare / Medicaid counseling.
- Successfully completing training and certification for IDOA programs.

Work Schedule : Monday thru Friday 9 : 00 A.M. to 5 : 00 P.M. Weekend / Evenings as needed

Minimum qualifications : Graduation from an accredited college or university with a Bachelor's degree in Public or Business Administration, Social Sciences, Planning or a related field, supplemented by three years of progressively responsible experience in program administration, or an equivalent combination of training and experience is required.

Preference will be given to candidates possessing the following :
