The Transport Caregiver reports to the Case Manager and is focused on transporting a client from appointments in Tyler to the client’s home and is responsible for ensuring safe, punctual, and comfortable transport for clients. This role involves assisting client in and out of vehicles, helping with mobility aids if needed, and providing companionship during the trips.
ROLES AND RESPONSIBILITIES – Directly & Through Subordinates :
- Safely transport client from appoinments in Tyler to the client’s home, ensuring punctuality and adherence to schedules.
- Assist client in and out of the vehicle, especially those with limited mobility, using appropriate equipment (e.g., wheelchairs, walkers) when necessary.
- Conduct regular inspections of the vehicle to ensure it is clean, well-maintained, and in safe working order. Report any maintenance needs or safety concerns promptly.
- Maintain accurate records of trips, including pick-up and drop-off times, mileage, and any incidents that occur during transport.
- Adhere to all local traffic laws, safety regulations, and company policies. Ensure all necessary licenses, certifications, and insurance are up to date.
- Provide a high level of customer service, ensuring that client feel comfortable and cared for during their journey. Address any concerns or issues professionally and promptly.
- Maintain clear and effective communication with dispatchers, medical facilities, and clients to coordinate schedules and address any changes or delays.
- May include early mornings, late evenings, weekends, and holidays depending on client needs.
- Primarily working in a vehicle with varying weather conditions. May involve occasional exposure to challenging situations or emergencies.

Perform other such tasks and assignments as directed by management / leadership of organization.
KNOWLEDGE, SKILLS AND ABILITIES
Previous experience in a driving or transportation role, particularly in a healthcare setting, is preferred.Ability to assist clients with mobility aids and to perform vehicle inspections and maintenance tasks.Strong interpersonal skills with a focus on empathy, patience, and clear communication.Familiarity with local roads, traffic patterns, and the locations of medical facilities.Must be available on 24 hours’ notice or less to transport a clientTRAVEL
Reliable vehicle and ability to drive at night.Must possess a valid driver’s license and up to date insurance and have a clean driving record.Normally, only local travel is expected for this position.PHYSICAL DEMANDS
