Description :
Schedule : Monday - Friday 8 : 00 AM - 5 : 00 PM, no weekends.
Typically, will work 40 hours per week.
If work is completed for the day, the temp will be sent home.
A Speech Language Pathologist (SLP) will evaluate and treat residents / patients with temporary or long-term speech, hearing, swallowing, and cognitive disabilities in compliance with state and federal guidelines.
They will evaluate the resident’s / patient’s speech, language, cognitive-communication, voice, swallowing, and fluency and develop treatment plans.
They will treat the residents / patients based on a physician’s treatment plan.
Essential Functions :
Screens and evaluates patients / residents and develops appropriate care plan.
Under a physician's direction, provides direct patient / resident treatment based on established plan of care.
Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
Administers, scores, and interprets specialized hearing and speech tests.
Instructs residents / patients, families, and caregivers to monitor speech and provide ways to practice new skills.
Completes required forms and clinical records in accordance with policy and state and / or federal regulations.
Documents in patient records in an accurate and timely manner.
Devises special tools and treatments for the specific needs of patients.
Provides comprehensive and compassionate therapy services in a sub-acute / skilled nursing facility setting.
Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
Maintains resident / patient privacy pursuant to regulations.
Ensures punctuality and regular attendance for assigned shifts.
Other Functions :
Develops treatment plans, including short and long-term goals and methods to achieve identified goals.
Communicates the resident’s / patient’s speech therapy program to the family and / or guardian.
Keeps written documentation including evaluations, daily notes and weekly summaries.
Performs other duties as assigned.
