About the Role

Heartland Nursing and Rehabilitation Center is seeking a compassionate and motivated Occupational Therapist to join our team on a PRN basis. In this role, you’ll empower residents to achieve greater independence and improve their quality of life through personalized therapy and support.

Key Responsibilities :

Conduct evaluations and create individualized treatment plans to meet residents’ functional goals.

Implement evidence-based interventions to promote independence in daily living activities.

Track and document resident progress, adjusting care plans as needed.

Collaborate with an interdisciplinary team to ensure quality, comprehensive care.

Support residents recovering from illness, injury, or disability.

Maintain accurate documentation and safeguard resident confidentiality.

Foster a positive, respectful, and compassionate care environment.

Qualifications :

Graduate of an accredited Occupational Therapy program.

Active Florida Occupational Therapist license.

Previous experience in long-term care or skilled nursing preferred.
