Job descriptionActive Registered Nurse license in the state of working. Bachelor's degree in Nursing or related field. Clinical Assessment, Medication Administration, and Treatment Planning skills. Minimum of two years of hospital, home health, or hospice care experience. Interdisciplinary Team Collaboration skills. Excellent communication and interpersonal skills. Ability to work effectively in a fast-paced environment. Driver's license, car insurance, and automobile for field work. Specialities in Medical / Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room. Floating expectations at Laguna Facility. All requests for time off (RTO) must be submitted upfront. Work schedule and shift type not specified.
Job Summary :
The Registered Nurse Case Manager provides skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. This role involves patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential for working with patients, providers, and team members.

Responsibilities :
- Provide skilled nursing care to patients.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Communicate effectively with patients, providers, and other team members.
- Manage patient cases, ensuring appropriate and timely care.
