Job Description
Delivers top quality care management services to identified patient populations through working collaboratively with the multidisciplinary team. Advocates, guides, and intervenes on behalf of patients, families, and / or caregivers. Engages identified patients in appropriate care management programs and provides patient centered interventions. Strives to positively influence the overall patient health outcomes as measured by clinical quality and utilization measures. Supports the achievement of system-wide population health goals within the framework of continuous improvement methodologies.
- Associate's Degree in Nursing required. BSN preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Certification in Care Management preferred.
- Strong professional level knowledge of comprehensive clinical assessment skills preferred.
- Knowledge of Medicare / Medicaid, insurance and regulatory guidelines is preferred.
- 3-5 years of relevant experience required.
