Fourth Street Clinic , in downtown Salt Lake City , has been providing essential health care services to those experiencing homelessness for nearly 35 years.

The Mobile Clinic Community Health Worker (CHW) is an essential healthcare advocate within the Mobile Clinic team. The Mobile CHW will bridge cultural and linguistic barriers, expand access to coverage and care, and improve health outcomes for patients in need.

Working on our state-of-the-art Mobile clinic, a 35-foot customized RV which was implemented in 2019 in response to the state's adjustment of their approach to homelessness which de-centralized shelters for the unhoused, the Mobile CHW will travel to various community partner locations, including homeless resource centers and permanent supportive housing centers, to assist our team in providing comprehensive primary care to people experiencing homelessness, particularly those who are unable to travel to our main clinic.

• Have experience collaborating across departments in an interdisciplinary setting to ensure delivery of medical, behavioral health, and support services
• Are proactive in recognizing needs and offering solutions

• Excel under pressure while maintaining organizational skills
• Have a passion for helping underserved patient populations
• Value trauma-informed care
• Speak Spanish (preferred)
