We specialize in deploying trained Community Health Workers (CHWs) to serve Health Plan Members (in-language) helping them navigate the complexities of the healthcare system and address social determinants of health. By establishing a trusting relationship, our CHWs empower Health Plan Members to take control of their health, leading to better long-term outcomes for both the individual and the community.
Position Summary
The Community Health Worker (CHW) Manager will play a critical role in the launch of a new CHW program in San Diego. This position is a hybrid role, combining hands-on community health work with the essential function of supervising a team of CHWs. The ideal candidate will be a compassionate, bilingual (English / Spanish) leader with deep lived experience and a profound understanding of the community's culture, needs, and existing resources.
The CHW Manager will be responsible for providing direct support to health plan members , focusing on improving health outcomes by addressing the social determinants of health and bridging the gap between healthcare systems and the community. You will lead, mentor, and empower your team to provide quality health education, navigation, and advocacy services, ensuring that members are connected to care and attend their preventive healthcare appointments.
Key Responsibilities
- Program Implementation & Supervision
- Supervise and manage a team of CHWs, providing regular one-on-one check-ins, case reviews, and performance evaluations.
- Develop policies and procedures and implement a work plan ensuring alignment with program goals and objectives and regulatory requirements from DHCS, etc.
- Conduct frequent monitoring of team activities to ensure quality improvement and adherence to organizational policies and ethical guidelines.
- Act as the primary point of contact for the team, addressing challenges, resolving conflicts, and providing support for complex client cases.
- Help recruit, hire, and onboard new CHWs, providing training and ongoing professional development opportunities.
- Direct Community Health Work
- Provide direct health education, health navigation, and advocacy to a diverse group of health plan members, with a strong focus on prevention.
- Work collaboratively with providers to ensure members receive comprehensive and culturally competent care.
- Engage with members to understand their barriers to care and address the social determinants of health such as language barriers, food security, housing, and transportation.
- Educate members on the importance of preventive healthcare, helping them schedule and attend appointments.
- Serve as a trusted liaison between health plan members and providers, advocating for the member's needs and ensuring their voice is heard.
- Maintain accurate and timely documentation of all client encounters and team activities in the organization's database.
- Community Engagement & Advocacy
- Build and maintain trusting relationships with community members, leaders, and partner organizations.
- Act as an advocate for health equity, working to address health disparities within the community.
- Collaborate with other agencies and stakeholders to strengthen referral pathways and coordinate care.
- Use data and member feedback to identify trends and advocate for systemic changes that address health disparities.
- Represent the organization at community meetings, coalitions, and events to promote the CHW program and the organization's mission.
Qualifications
College degree in health science, sociology, psychology, or equivalent work experience.Bilingual proficiency in Spanish and English is required.A minimum of 1-2 years of experience working as a CHW or in a similar community-based role.A minimum of 1-2 years of supervisory or team lead experience is preferred.Lived experience within the community served is highly desired.Experience with health navigation, health education, and advocacy.Deep knowledge of community resources and social services in San Diego.Excellent interpersonal, communication, and problem-solving skills.Demonstrated understanding of health disparities, cultural sensitivity, and the social determinants of health.Ability to work both independently and collaboratively as part of a multi-disciplinary team.Valid California Driver's License, car insurance, and reliable transportation.
