Nurse Practitioner or Physician Assistant

Hiring for multiple Locations : Erie area (Edinboro+Fairview), Corry, Warren, and Coudersport, PA

This position is ideal for for candidates with backgrounds in primary care, geriatrics, or long-term care who thrive in independent, relationship-driven clinical environments.

What You’ll Do

  • Provide comprehensive medical assessments and manage residents’ ongoing health needs.
  • Diagnose, prescribe, and adjust treatment plans to optimize patient outcomes.
  • Initiate and guide advance care planning conversations when appropriate.
  • Accurately document all encounters and ensure timely submission of billing information.
  • Collaborate with supervising physicians and participate in the on-call rotation as needed.
  • Partner with facility staff to enhance care quality and support ongoing education initiatives.

What You’ll Bring

  • Master’s degree and current Nurse Practitioner or Physician Assistant license (Family, Adult, or Geriatric focus).
  • 3+ years of clinical experience in Primary Care, Geriatrics, or Long-Term Care (preferred).
  • Strong diagnostic and decision-making skills with the ability to work autonomously.
  • Commitment to compassionate, patient-first care.
  • Reliable transportation, auto insurance, and compliance with all federal / state requirements.
