Location : Remote with occasional on-site member interactions

Schedule : Flexible rotating shifts between 8 : 00 AM - 8 : 00 PM CST, Monday through Friday

Reports to : Operations Team Supervisor and Manager

About the Role

Join our Clinical Engagement Services Center team, which delivers a wide range of clinical services including Adherence, Diabetes management, Fraud Waste & Abuse prevention, Medication Therapy Management (MTM), Retrospective Drug Utilization Review, and STARS adherence programs. Our team consists of clinical pharmacists, certified pharmacy technicians, and customer service representatives working collaboratively to meet operational and clinical goals.

As a Clinical Pharmacist, you will primarily conduct telephonic consultations with members to complete comprehensive medication reviews and improve disease state management. You will also document care provided and communicate essential information to members' primary care providers. Occasional in-person consultations may be required.

Key Responsibilities

• Engage members telephonically and schedule pharmacist consultations for clinical programs supported by the Clinical Engagement Services Center.
• Utilize prescription claims data to generate clinical recommendations in accordance with MTM and Retrospective Drug Utilization Review protocols.
• Provide recommendations related to gaps in care, high-risk medications, adherence, drug interactions, therapeutic and generic substitutions.
• Assess medication regimens for unnecessary or ineffective medications, inappropriate doses, and adverse drug reactions.
• Deliver patient education on medication regimens and disease states.
• Accurately document patient care notes and recommendations in a clear, concise, and professional manner.
• Stay current with emerging clinical trends.
• Participate in departmental meetings to improve programs and processes.
• Follow all policies and procedures and meet productivity and performance expectations.
• Support special projects as needed.
• Perform other duties as assigned by the Operations Manager.
