POSITION SUMMARY :

Responsible for supervision and training of individuals with developmental disabilities. Works closely with Case Manager & Program Manager to assist individuals in the development of their skills to their full potential, and foster their independence in a positive and caring manner. Completes required documentation.

DUTIES AND RESPONSIBILITIES :

• Use the established methodology for achieving client goals and track progress.
• Provides care, supervision, instruction and / or job coaching for the individuals we serve, services might be provided in person, remotely or in the community.
• Participates in weekly online videos.
• Development of activities, curriculum and schedules for programming in person or remotely.
• Assists the Case Manager in implementation of the Individual Service Plans (ISP's) / Individual Habilitation Component (IHC) and provides documentation as directed by the Program Manager.
• Assists with personal hygiene, toileting, feeding, and ambulation needs as needed.
• Accurately and legibly evaluates and documents the individuals’ progress through daily charting and log notes in client electronic file.
