Compensation : $232K$290K base + annual bonus Schedule : MondayFriday, no nights or weekends

100% post-acute rounding in partnered SNFs

Focus on prevention, chronic-condition management & readmission reduction

Requirements

MD or DO with active CA license

BC / BE in Internal Medicine, Family Medicine, or Geriatrics

Experience in post-acute or long-term care preferred

Skilled in goals-of-care conversations & documentation standards
