Advanced Practice Provider Palliative Care

Overview

Responsibilities :

As an outpatient palliative care advanced practice provider, you will provide compassionate and holistic care to patients with serious or life-limiting illnesses and their families. You will work collaboratively with a multidisciplinary team of physicians, nurses, social workers, chaplains, and other specialists to address the physical, emotional, spiritual, and social needs of patients and caregivers. A candidate for this position will :

• Perform comprehensive assessments
• Develop and implement individualized care plans, prescribe medications
• Order and interpret diagnostic tests, and provide education and counseling
• You will also participate in quality improvement initiatives, research, and education activities related to palliative care.
• Care will be provided at patients home
