Looking for a PER DIEM Speech Therapist for a Skilled Nursing Facility in Rochester, NY

Job R esponsibilities :

• Provide quality SLP services, including evaluating patients, screening , initiating referrals , obtaining data and follow through with plan of care.
• Completes timely documentation including progress notes, screen reports, daily notes, and discharge summaries.
• Knowledgeable in short term and long term SLP care .
• Instr uct patients as well as family members and staff in goals, progress, and appropriate therapeutic techniques.
• E nsure s compliance with protocols and regulations.
