Works with multi-disciplinary team to identify psychosocially complex patients. Perform psychosocial assessments of patients. Provide brief, crisis and supportive solution-focused counseling to patients / support systems to help address complex biopsychosocial concerns and adapting to complex, difficult and / or catastrophic illnesses and injuries. Early and appropriate identification of the need for Long Term Care Medicaid for patients to transition to the next level of care. Identify and refer to community resources to address psychosocial needs, including Social Determinants of Health, of varied inpatient populations. Provide assessment, consultation, coordination and referrals in situations of suspected adult abuse / neglect, and domestic violence. Work with multidisciplinary team to identify patients with limited capacity and the need for alternative decision makers through Surrogacy or guardianship.
