Job Description
GENERAL PURPOSE
Responsible for carrying out elements of the treatment plan and the provision of behavioral coaching to reduce crisis incidents. Most tasks are carried out in the community, especially family homes.
DISTINGUISHING CHARACTERISTICS
This is an at-will direct service position within a program. The position is responsible for assisting and advocating for our members in all areas of treatment and coaching our members to reduce crisis incidents.
ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)
The duties listed below are intended only as illustrations of the various types of work that could be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or a logical assignment to this class.
- Works in partnership with parents who are treated as experts concerning their families.
- Helps to conduct strength / needs assessment; assists with and participates in interdisciplinary team meetings.
- Completes all tasks associated with both fidelity to the START Model, program certification of the START Model, and individual certification in the START Model.
- Responsible for submitting / entering outcomes / incident data into identified database or providing this to person responsible for entering the information.
- Coordinates care as needed, internally and with outside agencies; Coordinates care with system partners.
- With the direct input from the child and family, helps to enlists the involvement of formal and informal community resources to support the family; sees the family weekly for periods of time / days that may vary depending on the need of the family.
- Keeps START Coordinator and supervisors informed of significant treatment or care coordination issues.

- Discharge planning / transitional service development; attends to the safety, health, and well-being of clients; responsible for all required record keeping as indicated by funder, certifying agency and TPCP.
- Safely transports clients and other passengers to and from appointments and activities.
- Provides prompt intervention in the event of a crisis both to stop the crisis and to notify when indicated, persons and agencies necessary for the resolution of the crisis situation; provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
- Is personally responsible and held accountable for work hours and time management as coordinated with the Team Leader.
- Provides support to other staff members and program members as needed.
- Provide after hours, “on-call” coverage to program clients on a rotational basis.
- Meets and maintains service productivity standards as set by the program; completes paperwork as assigned in a timely manner.
Qualifications :
A typical way of obtaining the knowledge, skills and abilities outlined above is through four (4) years of varied experience as a provider of mental health services or through graduation from a four (4) year college or university with a bachelor’s degree in Social Work or related field and two (2) years varied experience as a provider of mental health services. Additional qualifying mental health experience may be substituted for up to two years of education with Associates degree in a mental health related field and experience working with youth, Transition Aged Youth and Families.
Schedule : Monday – Friday, 8 : 30 am - 5 : 00 pm with the flexibility to work from 7 : 00 am - 7 : 00 pm
