Job description
A company is looking for a Care Coordinator (Bilingual Spanish, Medical Assistant) located in North Carolina.
Key Responsibilities
Manage provider schedules and prepare charts for home visit appointments
Conduct outreach for scheduling, appointment confirmations, and wellness checks for high-risk members
Document interactions and assist with medical records and referrals in the electronic medical records system
Required Qualifications, Training, and Education
Minimum 1 year of experience entering referrals and prior authorizations in a healthcare setting
High School Diploma or GED is required
Completion of a Medical Assistant program from an accredited institution is preferred
Knowledge of ICD9 and CPT codes and managed care plans
Bilingual in English and Spanish is preferred
