A - 47 beds
C - 50 beds (Covid unit, currently closed unit)
G - 59 needs
H - 40 bed secure dementia unit
J - 59 beds
K - 45 beds
EMR : PCC
Ratios : Days : 1 : 8-10; Evenings : 1 : 10-12; Nights : 2-4 CNA's per unit
Required Certifications : BLS, CNA license
Common diagnosis / Types of patients : Accept LTC, skilled, rehab and dementia residents 18+ years or older Hospice Care accepted in house - have 2 dedicated hospice rooms.
Job Duties : Vital Signs, passing meal trays and dining room assistance including feeding assistance / monitoring, I / O s, turning, bathing, changing the liens, tidy resident rooms as needed, all other ADL's as needed, activity / behavior / toilet charting, assist with oxygen as needed, rounding, shift report. Will assist with restorative needs (follow restorative care plans - walk, use adaptive equiptment, splinting, etc). Will assist / attend resident appointments as needed.
Experience or 'Must have' skills : Prior PCC experience (minimal training given).
Interdisciplinary Team : PT / OT / SLP, Pulmonary or Cardiac Rehab, Case Managers, Social Workers, Dieticians? PT, OT, SLP, Case Managers, Social Workers, Dieticians.
Is RT on-site 24 / 7 Oxygen Therapy / Equipment (ie Cpap, Bipap, High Flow) : CPAP, BiPap, O2
Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) : 8HR shifts including 30 min unpaid lunch break.
Weekend Requirements : Every other but able to work more if desired.
Holiday Requirements : YES. No set rotation, schedule is made out 2 weeks in advance and will work holiday if it falls on your scheduled day within the current schedule.
Call or Standby Requirement : No - will work scheduled shifts, don t send home early.
Floating Requirements : Try to keep in consistent areas but may be assigned to any area as needed.
Scrub Color / Dress Code : Any color clean, professional scrubs except camo.
