The Personal Care Assistant supports the mission, vision, and values of Golden Valley Health Centers Central Valley PACE Center by performing patient care activities and other tasks as assigned to assist the nursing staff.
THIS IS A HOME CARE POSITION.
Position will work at our Central Valley PACE Center in Modesto, CA.
Scheduled Monday - Friday, working from 8 : 00am - 5 : 00pm.
Compensation :
$22.19 - $23.29 an hour
Duties and Responsibilities
- Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.
- Communicates patient care needs to the LVN / Supervising RN and update Manager.
- May assist in Day Center and Activities when there is down time.
- Deliver DME to PRT's home and document in EMR.
- Flexible to work weekends when needed.
- Home Care Assessments.
- Communicates effectively with participants, families, staff, other departments and agencies.
- Works effectively in a team setting.
- Documents treatments / procedures, observations, nursing care and patient activities on appropriate forms.
- Participates in teaching / learning process with others.
- Follows guest relations policies and procedures.
- Abides by all established policies, rules and regulations.
- Attends staff meetings and takes part in participant care planning.
- Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
- Communicates effectively with participants, families, staff, other departments and agencies.
- Works effectively in a team setting.

- Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and / or physical frailties.
- Performs patient care activities appropriately, reporting any circumstances or change in patients condition.
