This contract will provide occupational medicine health services and electronic health records management for sworn staff from both the City of Phoenix (City) Fire and Police Departments as well as public safety personnel assigned to other jurisdictions throughout the region who have contracted with Phoenix Fire Department Health and Wellness Center (PFDHWC) for these services.

Location : Phoenix Fire Department Health and Wellness Center (PFDHWC)
50 S. 12th St., Phoenix, AZ 85034
PFDHWC Hours : Facility hours are 7 : 30 a.m. to 4 : 00 p.m. Monday through Friday. PFDHWC Clinic hours are 7 : 30 a.m. to 12 : 00 p.m. and 12 : 30 p.m. to 4 : 00 p.m. Monday through Friday, with a 30-minute lunch period.
Holiday Closures : The PFDHWC is closed on all City of Phoenix observed holidays.

This job is CONTINGENT upon contract award, client approval, completion of a favorable background investigation, and the ability to obtain and maintain a Public Trust / High Risk clearance.
Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
