Weekly hours may vary based on patient needs

Physical Therapist Requirements

Current Indiana Physical Therapist license in good standing
Graduate of an accredited physical therapy program
Previous home-health physical therapy experience required
Previous OASIS documentation experience required
Strong clinical-assessment and treatment-planning skills
Accurate and timely clinical documentation abilities
Knowledge of Medicare and home-health requirements
Excellent communication and interpersonal skills
Ability to work independently and manage a home-based caseload
Reliable transportation and ability to travel to patients’ homes
Compassionate, patient-centered approach to care

Key Responsibilities

Perform comprehensive physical therapy evaluations in patients’ homes.
Assess mobility, strength, balance, endurance, safety, and functional limitations.
Develop and implement individualized plans of care.
Provide skilled therapeutic interventions to improve strength, mobility, balance, endurance, and independence.
Conduct gait, transfer, and functional-mobility training.
Provide fall-prevention education and interventions.
Evaluate home environments and recommend appropriate safety modifications.
Educate patients and caregivers about therapeutic exercises and safe mobility techniques.
Complete OASIS assessments and required home-health documentation accurately and on time.
Monitor patient progress and modify treatment plans as clinically appropriate.
Communicate with physicians, nurses, occupational therapists, and other members of the interdisciplinary team.
Participate in care coordination and discharge planning.
Provide care consistent with Medicare, home-health, and agency requirements.
Perform additional physical therapy duties as assigned.
