As a CHW, youll conduct screenings for health-related social needs, help patients navigate complex systems, and create individualized care plans that address barriers such as housing instability, food insecurity, transportation challenges, and access to community programs. Youll engage directly with patients in their homes and communities, often building long-term, trusting relationships that empower them to achieve their personal health goals.
