Care Manager LCSW at Providence Mission Hospital in Mission Viejo, CA. This position is Full-time and will work 8-hour, Day shifts.

Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. Required Qualifications :

• Education to meet certification, license or registration requirement.
• Upon hire : California Clinical Social Worker License
• 6 months of Clinical experience in an HMO, medical group, affiliated model, hospital or medical office / clinic setting.
