Competitive Pay : From approximately $90,000 to $136,000 per year, based on years of experience.

40 hours per week

Purpose :

Serves as an advanced practice nurse providing primary level of care in collaboration with physicians and other members of the health care team.

Participates in the care of home-based / facility-based / palliative care patients in an independent and interdependent role.

Responsibilities :

  • Obtains patient histories and performs physical examinations; prescribes appropriately based upon clinical findings.
  • Works collaboratively with members of the interdisciplinary group in a way that fosters holistic care.
  • Collaborates with primary physicians to determine the plan of care for a patient whether the patient is enrolled in palliative care or hospice.
  • Maintains existing relationships and establishes new referral relationships with community physicians and healthcare systems / personal to ensure appropriate management and care of patients referred to hospice and palliative care as well as to foster education and awareness of EveryStep services.

Confers with referring physicians over the telephone or per office visit to update orders or patient's physical condition.

Provides written and / or verbal feedback regarding patient care.

  • Manages care of a palliative care or hospice patient in the home, hospital, hospice inpatient units or nursing homes. Establishes routine home, clinic, hospital and nursing home visit schedule for assigned patients.
  • Provides leadership for creation and development of specialized hospice and palliative care services.
  • Possesses sufficient self-awareness to maintain professional boundaries while accompanying vulnerable patients and families during intense times in their lives.

  • Commits to active participation in Quality Assessment and Performance Improvement initiatives of the organization.
  • Participates in on-call coverage with the interdisciplinary group members as needed.
  • Understands the difference between hospice and palliative care. Education will be specific to the type of care being provided.
  • Collaborates and assists education department in development and deployment of educational curriculum for both onboarding and continuing education of staff

Palliative Care :

  • Makes palliative care consultation visits in the nursing facilities, hospitals and homes when a consultation is ordered by a physician.
  • Documents and codes the palliative care consult visits appropriately.

Hospice Care :

Effectively assess and actively participates in constructing a comprehensive plan of care for pain and symptom management for patients enrolled in hospice care and their families.

Qualifications :

  • Graduate of an accredited school offering an advanced degree in nurse practice (ARNP)
  • Unrestricted license to practice within the State of Iowa
  • At least 2 years related experience preferred
  • Hospice or palliative care experience preferred
  • Ability to maintain confidentiality and protect sensitive data at all times
  • Ability to work collaboratively and support a team environment
  • Ability to provide exceptional customer service and to interact effectively with team members and the broader health care community
  • Strong interpersonal skills, communication, collaboration and conflict resolution
  • The ability to manage to multitask and effectively handle the emotional stress of the workload
  • Current DEA certificate without restriction
  • State CSA
  • Carries professional liability insurance coverage : minimum limits
  • Current CPR certification
  • Must have valid driver's license, auto liability insurance and reliable transportation if driving is required
  • Successful completion of pre-employment physical, drug screen, 2-step TB test and background check

Working Conditions :

  • Physical requirements : exerts up to 50 pounds of force occasionally, and / or up to 20 pounds of force frequently, and / or up to 10 pounds of force constantly to move objects
  • Physical activities : kneeling, talking, hearing, standing, walking, visual activity
  • Environment : atmospheric conditions (fumes, odors, dusts, mists, gases)
  • Time : Work hours based on patient-driven census model
  • Travel : Required between locations
