Group Vice President Of Chronic Kidney Disease And Value Based Care Capabilities

DaVita Integrated Kidney Care is seeking a Group Vice President of Chronic Kidney Disease and Value Based Care Capabilities (CKD and VBC Capabilities). DaVita Integrated Kidney Care (DaVita IKC) is one of the largest end-to-end kidney care providers; we manage $5.5B+ in annualized medical spend through value-based partnerships. We provide comprehensive care and complex chronic condition management to 80k+ patients nationwide, whether they are in the early stages of kidney disease, transitioning to dialysis, seeking a kidney transplant or receiving life-sustaining dialysis.

Essential Duties and Responsibilities :

• Reporting to the Chief Transformation Officer, the Group Vice President will support DaVita IKC's executive leadership team in achieving strategic goals with overall accountability and responsibility for Patient Engagement, Physician engagement, and Total Cost of Care Reduction for CKD patients via advanced primary care.
• Collaborate with the Chief Medical Officer and Group Vice President of ESKD Models of Care to deliver clinical innovations and excellent patient care.
• Accountable for engaging nephrologists in the DaVita IKC model of care via our high performing physician network (includes over 2400 nephrologists).
• Lead the continued innovation and development of our CKD models of care.
